Please read the following disclosures:

The undersigned Authorizing Agent(s) hereby certify that they are legal custodian(s) of the below named Deceased, having full legal authority to authorize cremation, processing and disposition of the cremated remains of the Deceased, and hereby authorize KRAMER FUNERAL HOME to take possession of and make arrangements for the cremation, processing and disposition of the remains of the below identified decedent, the Authorizing Agent(s) represent and certify that the remains  delivered for the cremation are those of the Deceased and the Authorizing Agent(s) further represent that they have the right to control the disposition of such remains.


As Authorizing Agent, I have read and understand the description of the cremation process contained  on the reverse side and authorize the cremation of the remains of the Deceased.  I further authorize KRAMER FUNERAL HOME to deliver the Deceased to the crematory for the purpose of cremation.

If you are signing this authorization in conjunction with others (i.e. siblings) who have signed a paper copy of this form at the funeral home, we will advise you if they have authorized any witnesses. Your authorizations must match.

Following the cremation, the Authorizing Agent directs KRAMER FUNERAL HOME to undertake the actions set forth on the reverse for final disposition of the cremated remains of the deceased.  If the cremated remains are to be shipped, the funeral home will only do this via USPS Certified Overnight Mail with return receipt, as a method of tracking the status and location of the cremated remains. If the Authorizing Agent fails to retrieve the cremated remains from the funeral home within (90) days of this authorization, the funeral home is instructed to dispose of such cremated remains in accordance with WAC 38-47-070, including but not limited to (a) releasing the urn for burial at a state or national veteran’s cemetery (if eligible); (b) interring such urn in a common plot designated for unclaimed urns at the Colfax Cemetery (Colfax, Washington); or (c) scattering the ashes at a place designated by the funeral home.  Only one notification to claim the urn will be made to the Authorizing Agent before the funeral home will take action on final disposition.

The Authorizing Agent acknowledges that that KRAMER FUNERAL HOME and the crematory are relying upon the representations being made by the Authorizing Agent in this authorization.  The Authorizing Agent agrees to indemnify and hold harmless KRAMER FUNERAL HOME and the crematory, their officers, directors, employees and agents from any and all claims, demands, actions, causes of action or suits of any kind or nature whatsoever, including, but not limited to, any legal fees arising out of, or resulting from the reliance on, or performance consistent with, the directions, statements, representations and agreements contained in this authorization.

The Authorizing Agent(s) represent and certify that the remains delivered for the cremation are those of the Deceased and the Authorizing Agent(s) further represent that they have the right to control the disposition of such remains.
Further, I understand and agree that I have read and understand the sections of the PDF form that are not included on this electronic form and agree to those terms as well.
